Activity Sign-In Forms & Reports

The following are the only currently accepted Activity Sign-In Forms for EBC events: These are to be either filled out at, or ahead of, the event, and submitted in a manner that the form provides.  In all cases, the submitted form must be in the correct hands ( MemberMiles@ElmhurstBicycling.org ) within a week of the event.

Online-only.

• Event-specific, guest (all non-members) Release & Waiver E-Form    Automated email submission to club Membership Coordinator. 

Fill in online, print, scan, then email.  ...Or download/print, fill in by hand, scan, then email.

• Combination (2-page), Member Activity Form & Guest Activity Release & Waiver   Ride Leaders need to bring at least one copy of both pages of this form to every EBC event. /  You can fill this form and print without saving, but if you want to save often repeating data (such as Ride Leader contact info), then download this as your original and always open and save as new copy before entering updated member data on the new document. This will preserve your personal version of the form for your future use.
• Member Activity Form pdf only   Just the activity sheet page.
• Guest Activity Release & Waiver only   Just the waiver page.
• Incident / Accident Report, PDF format   Should there be an accident or other significant incident on an EBC event, the following report is to be filled out and sent to the Ride Captain ( RideCaptain@ElmhurstBicycling.org ).  This form is only accessible to logged in members.

Simple, printable business cards

• EBC Business Cards: *monochrome*  Print your own page, on your favorite paper and cut out 10 business cards to carry with you.
• EBC Business Cards:  *color elements, clear background*  Like above, except color text and graphics. Best printed on color printer on a white page.
• EBC Business Cards:  *full page color*   Same as color elements version above except full background color is included. Best for professional prints. Will use a lot of ink.
